Arizona Wellness Medicine

Dr. Emily Parke works with your current healthcare team and is not considered a replacement for your primary care physician. All new patients are required to have a current primary care physician to cover emergencies, routine care, and screening.

Functional Medicine consultation services may or may not be covered by your insurance plan. Please check with your insurance prior to your visit. Our office does not bill your insurance directly but we will give you a receipt for services, which you may submit for reimbursement. We are not a participating provider in Medicaid or Medicare.

Arizona Wellness Medicine operates as a self-pay clinic, and payment is expected at the time of service. If you have any questions, please feel free to contact us.

Expect to spend approximately 120 minutes at the office for your initial visit, including checkout time. You will spend 75-90 min of face-to-face time with your physician. The initial consult fee includes time spent before and after your visit to review any past laboratory studies provided and all paperwork and intake forms.

Your first follow up visit is 60-minutes in length, as it will include review of your case and results, assessment of progress, as well as formulation of your plan moving forward.  Subsequent follow-up visits are 45-minutes in length. However, a longer follow-up visit for complex discussions and extra time to review lab work may be booked as you and/or your physician see fit, and will be charged accordingly.

Arizona Wellness Medicine offers a variety of packages, to meet the needs of all patients.  All packages must be used within 12-months of purchase.

The Institute for Functional Medicine teaches practitioners how to assess the patient’s status relating to modifiability lifestyle factors and fundamental clinical imbalances through careful history taking, physical examination, and laboratory testing. As Functional Medicine Practitioners we look at body systems and address clinical imbalances.
