Can I become a functional medicine patient if I live outside the Phoenix/Scottsdale area?
At this time we require an in-person initial consultation to establish care, and are able to do follow up visits via phone or video conferencing.
Does Arizona Wellness Medicine accept health insurance?
Although we do not deal directly with insurance companies, we are happy to provide you with an insurance invoice that you may turn into your insurance company for possible reimbursement or credit towards your deductible. We cannot guarantee that your insurance carrier will reimburse you for our services, as that is up to your insurance company.
Will my health insurance pay for any lab testing?
There are 2 categories of testing that we offer, conventional lab testing, and functional medicine lab testing. With conventional lab testing, you have the option of going directly through your Insurance, to whichever lab your insurance prefers, as we have accounts with both Sonora Quest and LabCorp locally. We cannot guarantee what coverage will be, as this is determined by your insurance. We also have cash pay options available for those with no insurance, or with high deductibles that want to pay out of pocket up front. It is also possible that your insurance will cover part of your functional medicine laboratory investigations as well.
Can I use a Health Savings Account (HSA) or Flex Spending Account to pay for my visits, testing, and supplements?
We do accept credit cards for payments, including HSA or FSA based credit cards, however, you will need to check with your HSA or FSA provider to determine what services and charges are covered.
What is the average cost for a new patient consultation, including labs and supplements?
Every patient is different, thus the cost of your first visit will depend heavily on the amount of functional medicine testing and supplements recommended. Please note that the functional medicine testing is a critical part of the process in finding and treating the root causes of your symptoms.
How many visits will I need?
Functional Medicine is a highly personalized approach to medicine, thus we cannot tell you exactly how many visits it will take to get better. After your first visit we will give you an estimate as to how many visits you may possibly require based on the initial assessment. As a general rule, the more symptoms and diagnoses you have, the longer it will take to uncover all of the root causes and treat them.
Do you have financing or payment plans?
Payment is expected at the time of service, and may be made in the form of cash or credit card. Although we do not directly have payment plans, we are partnered with Advanced Care Credit, a third party financing company that allows you to apply for credit, and if approved, will send you a credit card that you can use only at Arizona Wellness Medicine, which you may apply to future visits. The percent APR you receive will largely depend on your credit score, and is directly determined by Advanced Care Credit.
What is your cancellation policy?
We require a minimum of 3 business days’ notice for all appointment cancellations and reschedules, to allow someone else to fill your spot. A $100 fee will be charged for cancellations with less than 3 business days notice, as your practitioner has already begun reviewing your chart and spending time on your case. The full cost of your appointment will be charged for cancellations with less than 24 hours notice, and for no shows also.
What is the age range of patients seen at Arizona Wellness Medicine?
We see patients from age 5 on up.
Do I need to keep seeing my primary care physician or will Arizona Wellness Medicine be my primary care physician?
Arizona Wellness Medicine, is a consultative functional medicine and IV Therapy practice, and is not meant to replace your current primary care or any other specialty physician. Although we will be able to address most of your primary care concerns, we do require that you maintain your primary care doctor for routine screening exams such as pap smears and prostate exams for example, and for urgent or emergency situations requiring immediate care or hospitalization, as we do not provide on call services.
What happens after I am better?
Most patients will be advised to come in for a visit every 6 to 12 months for follow up once you are better, so we can remain proactive and prevent illness and symptoms from occurring. Our goal at Arizona Wellness Medicine is to get you to a point where you have control over your health, thus minimizing the need for us, or any other health care providers.
